Healthiest Nuts to Eat: RCT Evidence and Rankings | Nut Cravings
Every "healthiest nuts" article gives you the same list with the same generic bullets. Almonds for vitamin E, walnuts for omega-3, Brazil nuts for selenium. All true, none complete. What those articles don't tell you is how large the mortality evidence actually is, which nuts won head-to-head in network meta-analyses, what the PREDIMED trial actually found, and why picking one nut to eat every day is a less evidence-supported strategy than rotating through several. This guide covers what the clinical data shows, nut by nut, benefit by benefit, without ranking them by opinion.
No single nut is the healthiest. A 2025 dose-response meta-analysis (63 studies) found highest vs lowest nut intake associated with 23% lower all-cause mortality and 26% lower CVD mortality. The PREDIMED RCT found mixed nuts reduced primary CVD events by 28%. How large is the nut mortality evidence?
Before comparing individual nuts, the aggregate evidence deserves a number most articles skip. A 2025 systematic review and dose-response meta-analysis pulled 33 publications covering 63 prospective cohort studies, searching Web of Science, Embase, and PubMed through October 2023. Comparing highest vs lowest nut intake categories, the results were:
These are observational associations adjusted for confounders, not RCT results, with the exception of PREDIMED. The dose-response relationship is linear up to roughly 2oz per day, after which the benefit curve starts to plateau. Each additional 28 grams per week was associated with RR 0.96 for CHD (95% CI 0.95-0.97), 0.90 for CVD mortality (0.86-0.94), and 0.92 for all-cause mortality (0.90-0.94). The PREDIMED trial is particularly important because it's the rare RCT that measured cardiovascular events rather than just biomarkers. It enrolled 7,447 adults at high cardiovascular risk and randomized them to a Mediterranean diet plus mixed nuts (walnuts, almonds, hazelnuts), a Mediterranean diet plus olive oil, or a control diet. The mixed-nut group had a 28% reduction in primary composite CVD events (myocardial infarction, stroke, cardiovascular death) vs control, compared to 30% for the olive oil group.
Best nut for each health goal: what the RCTs actually show
Rather than ranking nuts from best to worst, which requires arbitrary weighting, this guide maps the strongest clinical evidence to specific health goals. Most readers are interested in nuts for one or two particular reasons. Here's where each nut leads the evidence base.
A head-to-head network meta-analysis of 34 randomized trials comparing pistachio, walnut, almond, hazelnut, and cashew-enriched diets against control and against each other using SUCRA rankings placed the pistachio-enriched diet first for total cholesterol reduction (SUCRA 96%), LDL reduction (SUCRA 87%), and triglyceride reduction (SUCRA 85%). Walnuts ranked second for TG and TC. No other nut ranked first across all three lipid outcomes simultaneously.
The mechanism is pistachios' status as the highest-phytosterol tree nut (primarily beta-sitosterol), which competes with dietary cholesterol for absorption at the intestinal brush border via the NPC1L1 transporter. In specific RCTs at 1.5oz per day, LDL was reduced approximately 6% and oxidized-LDL also decreased significantly.
The WAHA trial enrolled 634 older adults and randomized them to a walnut-supplemented diet (30-60g/day) vs a walnut-free control diet for two years. The walnut group showed significant reductions in 6 of 10 measured inflammatory biomarkers: IL-1beta, IL-6, TNF-alpha, hs-CRP, sE-selectin, and sVCAM-1. This is a two-year RCT, not a six-week feeding study, which makes the findings considerably more clinically meaningful.
The mechanism involves walnuts' 2.5g ALA omega-3 per ounce (the only tree nut with meaningful omega-3 content), ellagitannins that ferment in the gut to produce urolithins with NF-kB inhibitory activity, and melatonin (one of few whole foods with measurable melatonin). A separate 2026 meta-analysis of 49 RCTs (4,611 participants) confirmed LDL reduced by 5.68 mg/dL, total cholesterol by 6.39 mg/dL, and triglycerides by 6.24 mg/dL.
A 2025 meta-analysis of 36 almond-specific RCTs (2,485 participants) confirmed: LDL reduced by 5.1 mg/dL, total cholesterol by 6.2 mg/dL, non-HDL cholesterol by 7.9 mg/dL, and ApoB by 4.6 mg/dL (all p less than 0.001). This is one of the largest nut-specific lipid meta-analyses published. Almonds are also the only nut to reduce both body mass and fat mass in meta-analysis.
Two UC Davis RCTs confirmed almonds improved wrinkle severity (-9% to -16%) and skin pigmentation over 16-24 weeks, driven by almonds' alpha-tocopherol vitamin E content (49% DV per ounce, highest tree nut). The King's College London 2022 RCT found 56g/day almonds for 4 weeks significantly increased fecal butyrate compared to a muffin control, confirming prebiotic gut effects.
One Brazil nut can contain between 11% and 288% of the daily selenium requirement depending on the soil region in the Amazon where it grew, with an average of about 95-100 mcg per nut. No other whole food delivers a comparable selenium dose in such a small serving. The selenium form is organic selenomethionine, more bioavailable than inorganic selenium supplements.
A meta-analysis of 7 RCTs (315 participants) found Brazil nut consumption increased plasma selenium by 87.63 mcg/L and significantly increased GPx (glutathione peroxidase) enzyme activity, reflecting functional selenoprotein upregulation. Selenium is required for 25 selenoproteins including the DIO1/DIO2 enzymes that convert T4 to active T3 thyroid hormone, GPx family antioxidant enzymes, and SELENOP which transports selenium to the brain. The dosing rule: 1-3 per day covers the benefit without approaching the 400 mcg/day upper limit above which selenium toxicity becomes a risk.
Cashews provide 69% DV copper per ounce, the highest of any common tree nut. Copper is required for cytochrome c oxidase (ATP synthesis), Cu-Zn superoxide dismutase (antioxidant defense), and ceruloplasmin (the ferroxidase that enables iron release from cells). Copper deficiency causes iron-deficiency anemia that doesn't respond to iron supplementation because ceruloplasmin is required to mobilize stored iron.
The 2024 Brazilian Nuts Study RCT (68 adults with overweight/obesity, 8-week, 3-arm randomized controlled trial) found the cashew group significantly reduced liver enzymes AST (p=0.007) and ALT (p less than 0.001) and ApoB by 6.6 mg/dL (p=0.003), while the control group showed no significant changes in either marker. The liver enzyme findings are novel: elevated AST and ALT are markers of hepatocellular stress associated with NAFLD, insulin resistance, and metabolic syndrome.
Pistachios appear twice in this guide because they lead in two separate categories. For eye health, they're in a category of one: the only common tree nut containing substantial amounts of lutein (approximately 290 mcg per ounce). Lutein and zeaxanthin are xanthophyll carotenoids that accumulate exclusively in the macula of the retina, forming the macular pigment that filters harmful blue light and protects photoreceptors from oxidative damage.
The 2024 Tufts University RCT enrolled 36 healthy adults with habitually low lutein intake, randomized to 2oz pistachios daily vs usual diet for 12 weeks. Macular pigment optical density (MPOD) increased significantly in the pistachio group by week 6 and was maintained through week 12. Control group: no change. Serum lutein also increased significantly. Higher MPOD is associated with reduced risk of age-related macular degeneration, the leading cause of blindness in older adults.
Why rotation beats eating the same nut every day
Most nut health articles implicitly recommend eating one "best" nut daily. The evidence doesn't support this. Each nut has a genuinely distinct nutrient profile that no other nut fully replicates. Here's the non-overlapping contribution of each:

The PREDIMED trial's mixed-nut arm used walnuts, almonds, and hazelnuts together, not a single nut, and this combination produced the 28% CVD event reduction. Do nuts cause weight gain?
This concern keeps more people from daily nut consumption than almost any other factor. The evidence consistently says no. Multiple mechanisms explain why despite being calorie-dense, nuts don't translate to weight gain in the research literature.
The caloric discount. A USDA-funded study found that whole almonds provide approximately 21% fewer calories than the Atwater standard suggests, because fat trapped in intact nut cell walls is not fully absorbed. This caloric discount disappears with almond butter, almond flour, or any processed form where cell walls are disrupted. The same principle applies to other whole raw nuts to varying degrees.
Satiety and subsequent intake. The fat, protein, and fiber combination in nuts generates stronger satiety signaling than equivalent calorie snacks. An 8-week randomized controlled trial (48 participants) found mixed-nut consumption vs pretzels resulted in significantly lower body weight (p=0.024) and BMI (p=0.043) at the same calorie allocation. What the meta-analyses confirm. Multiple systematic reviews conclude that nut consumption does not promote weight gain in humans. The caveat: these findings apply to whole nuts consumed as a snack replacement, not as calories added on top of an unchanged diet. If you eat an ounce of nuts daily and reduce another snack by equivalent calories, the neutral or beneficial effect on weight is well-established. If you add nuts to everything you're already eating without adjustment, calorie mathematics still apply.

Malondialdehyde, lipid hydroperoxides, and other lipid peroxidation products generated in oxidized nuts are not neutral compounds. Walnuts are particularly vulnerable: their high PUFA content (about 47% polyunsaturated fat) oxidizes faster than almonds or cashews. Pistachios' lutein degrades with heat and oxygen exposure. The proanthocyanidins in hazelnuts are also sensitive to heat and time. Regional grocery store nuts often arrive weeks to months after processing, with unknown cold-chain handling in between.
This isn't a theoretical concern.
